Ventilator and post sub acute care:
A select few of our facilities offer ventilator units on site. With a Certified Respitory Therapist on staff and a skilled nursing staff they can meet the needs of patients with this critical need. Post subacute care such as IV’s, Tracheostomy care and other skilled services can be met at most of our facilities. Make sure you have this discussion with Admissions staff to ensure the facility can meet your needs or they can help direct you to one of our other facilities that can.
xPeritoneal and “In Home” hemodialysis:
Some Provider Services facilities can provide Dialysis on site in their comfortable clinics with expert staff. All of our facilities can arrange transportation and get you to the best clinic that meets your needs in the immediate area.
xBariatric Care:
Provider Services has specific homes that work specifically with Bariatric Patient, from a licensed Dietician and a staff of counselors we can help you manage this disease to get you healthier and more independent. All of our homes can manage the needs of most Bariatric patients with special equipment and dietary needs.
xI.V. and TPN therapies:
Provider Services skilled nursing facilities can help manage I.V. care and Total Parenteral Nutrition with a qualified staff of RN’s, licensed Pharmacists and Licensed Dieticians.
xSecured units for Memory Care:
Provider Services has specific facilities that provide and set the standard for Memory care in Ohio. From secured units, to specially trained staff, and specific activity programming we are here to meet the needs of Resident with Alzheimers disease and other related dementias.
xComprehensive Rehabilitation:
From a broken hip to a knee replacement our facilities are equipped with the most up to date equipment and services to get you back on your feet as quickly as possible. Our staffs of Physical, Occupational and Speech therapist are ready to get you going again. Each facility employs it own therapists so that gives you the patient the same people to work with day in and
xWound Care and Ostomy Management:
Provider Services teams up with Nurse Practioner wound specialist to provide in-house treatment of wounds and management of Ostomy sights. The team will work with your physician to provide you with the best treatment and healing protocol.
xSecured Behavioral Rehabilitation Units:
Certain Provider Services Homes specialize in behavioral rehabilitation and care. We offer Secured Behavioral units, Men only units and behavioral only homes are there for people with this special need. With a team of Psychiatrists, Psychologists and counselors to help manage behavioral based diseases.
xRespite Care:
Provider Services understands that not everyone needs permanent nursing home placement but also understands that the caregivers may need a break. We offer respite stays from a couple days to a couple of months. This gives you the caregiver the opportunity to become refreshed and recharged. Please ask the admissions team at the facility you visit to see what their specific respite policy is.
xHospice Care:
Provider Services teams up with Tridia Hospice to provide any Resident facing an end of life situation with dignity, grace, love and peace.

Person Centered Care:
Person centered care is a practice that puts the facilities daily routine around the Resident instead of the Residents daily routine around the facility. We want you to tell us when you want to get up, go to bed, when to take your shower, eat your meals and what activities you enjoy best. Person centered care is constantly changing and evolving to make your stay at one of our facilities the best it can be.
